
Appointments:
To schedule an appointment please call (414) 385-2700. We will register you by asking information about you and your insurance. Prior to your visit you will be mailed an registration packet. It will contain several pages of Initial Visit Forms. They will ask questions regarding your current condition and past medical history. The information is helpful in assisting your physician with making a diagnosis and choosing the appropriate treatment options for your condition. Please have these forms completed before your appointment. If your appointment is to be scheduled very soon, you may not be able to receive the packet from Integrated Spine Care in time. If this occurs, please download the above form and bring it to your appointment completed. If you are unable to download the forms, we ask that you arrive at least 20 minutes before your appointment time.
Your doctor will be reviewing all prior medical history, particularly related to your present spine condition. To assist in this comprehensive evaluation, you will need to provide the following documents and studies from all prior evaluations or treatments that you may have had.
These could include:
- All radiology films and reports ie. MRI, CT, Myelogram, Discography
- X-ray, bone scans etc.
- Other tests (EMG, Injections, etc.)
- Any prior surgical reports
- Other treatment notes from physicians, chiropractors, therapists
What to Expect:
Remember you will need to bring the ACTUAL FILMS of MRI, CT, and x-rays to your appointment. Please ask the imaging facility you go to provide films vs CD’s if possible.
A common reason for rescheduling appointments is due to patients arriving without the above. Please note that Integrated Spine Care does not have direct access to the above information from other providers, however, if your diagnostic studies were performed at St. Luke's Medical Center, Integrated Spine Care can obtain the studies and the reports for you. It is a good idea to call Integrated Spine Care in advance to confirm that all studies and records have arrived, especially if they have been mailed from another facility. If you have difficulty obtaining the above information, please call our office for assistance.

Insurance Coverage:
We welcome you as our patient and are happy to assist you with insurance benefit information and the plans we participate in. Even if your insurance company does not have an agreement with Integrated Spine Care, you still may be eligible to see us. Because of the complexities of insurance plans and benefits, we will assist you as best we can with these insurance questions.
(Note, in many cases your insurance will pay a portion of your bill, but your share maybe greater. Some insurance plans may not pay any of the bill leaving the patient directly responsible for the entire amount. Your insurance plan or Integrated Spine Care may request that a portion of your bill be paid at the time of your visit. While we want to help with benefit information, it is ultimately your responsibility to determine your specific plan benefits in order to understand what costs may or may not be covered.) We ask that you pay your co-pays at the time of your visit.
Prescription Renewal Policy:
Medication prescription renewals will be provided during normal business hours. Please allow 72 hours or 3 business days for renewals. Please plan ahead for weekends and holidays accordingly. Medication renewals will not be provided during non-business hours.
Confidentiality:
Your medical information is confidential. Generally it can be given only with your written permission. If you wish for us to release your medical records, Integrated Spine Care will provide you with the "Release of Information" form to facilitate record transfer.
Note: We do not see patients who have been involved in a motor vehicle or personal injury accident.
Work Comp Injuries:
We gladly accept patients who have had work related injuries. Our physicians have expertise in caring for work injuries relating to the spine. At Integrated Spine Care we will always provide the first available appointment. We understand the importance of assessing the injury and providing a treatment plan which will achieve a successful outcome. We will provide necessary reports for your workers' compensation claim. You will need you to provide your workers' compensation claim information to our office. You will also need to provide your regular insurance information. Sometimes workers' compensation claims are denied and your regular health insurance will provide benefits instead. Our office and our physicians will provide assistance with your workers' compensation claim.
Case Managers
At Integrated Spine Care, we work closely with the injured patient, case manager, insurance carrier and employer. Case Managers are welcome to attend appointments with their clients. Please use our Case Manager room when you visit us!
